My car was running sluggish and sounded like it was "missing" at idle. I plugged in the OBD2 and got the following.

P0021 - "A" camshaft position - timing over-advanced or system performance (bank 2)

The web link further explains it as

The "A" camshaft shall be either intake left or front camshaft. Bank 1 contains cylinder number 1' bank 2 is the opposite bank

Anyone have any ideas what could be wrong and/or a fix?

If you don't have a VAG.com (Ross-Tech: VCDS) system (a generic OBD-II scanner will not allow you to invoke the necessary diagnostic programs enbedded in the ECU) and the Bentley Audi service manual (Audi B6 S4 V8 > Maintenance > Manuals > ES#6277 Bentley Service Manual On CD - AB66) for your vehichle (the "gold standard" for Audi service manuals), you could do some serious damage to the engine by poking-around...

The S4's camshaft-adjuster can be diagnostically exercised; the procedure is outlined in the Bentley service manual. You'll need a "real" VAG.com diagnostic system to prompt the ECU to test and report on the camshaft adjuster's operation.

Good luck!
