98 cab driver door handle

i am having problems with the driver-side door handle (interior). the little box that surrounds the actual handle has come loose and the whole assembly moves when you pull the chrome handle. you can still release the door but you have to hold the assembly against the door while pulling the chrome handle.

any ideas of how to fix? why the assembly has come loose? thanks in advance.
